Different quake games call it different things, but yes "OpenGL 3Dfx" is what you want.
You also need to be running the 1.48 miniport.
You having this problem in any other games? Overheating is a possibility. Are you overclocking?

I haven't installed any other games on this rig since the reformat so I'm not sure if it's just a quake2 problem or not. Also, I'm not overclocking and even have one of the old 3 Finger's Kick Ass Cooling Fans pointed at the card so I'm pretty sure that it's not over heating.
Actually, at this point I'm tempted to just upgrade to a newer card to see if it fixes the problem but don't really want to spend much more than $50. This should probably be a whole new post but... got any suggestions on a new/slightly used card for around $50?

yea, Q2 worked great and the worst part of it is, I just got a cable modem a few months ago where now, instead of pinging servers at +250, I'm coming in under 100 but usually can't stay in a game long enough to enjoy it. Some nights however, I can play up to 20 minutes or so before getting booted. Maybe the card IS overheating... I just don't know.

Those Voodoo 3 card run very hot. I was really surprised how hot that heat sink got. You could easly fry an egg on it. To rule out heat, remove the side cover and place a house fan blowing air inside the case. Then try Q2 to see if that helps. Also note that just because you have a fan blowing on the card, if you don't have the cooling setup correctly, the hot air will just circulate inside the case.

Okay, try some things to narrow it down:
- In Q2 video mode select "Software" instead of "OpenGL 3Dfx". See if you can play without getting booted. That will confirm it's a 3D related problem.
- Try disabling the sound card and see if you get booted (it's just as likely this is a sound driver issue and we've ignored this so far). Type this in the Q2 console to disable sound for that session (when you restart Q2 it'll be back on):
set s_initsound 0
snd_restart
If it works okay with the sound disabled then update your sound drivers to the latest version. If you have an SBLive you might have an IRQ sharing issue or an IRQ PCI Channel sharing issue (I doubt the later since it was working before the format, but the former is possible since this install of Windows might be allocating the IRQs differently). Make sure that the sound card (especially if it's an SBLive) is not sharing an IRQ.

Well,
After following Flying Penguin's advice I found out 2 things:
1) Quake II was never designed to be played in software mode
, and
2) my problem's are most definitely 3D related.
In Open GL mode I couldn't play online for more than 45 seconds without getting kicked but after switching to software mode I played for over 20 minutes straight without a problem (other than the fact that I couldn't see worth a $%@#)
I guess now I need to figure out if it's driver related or heat related. Because of the location of my case it's kind of difficult to open it up and point a fan at it as Executioner has suggested so that will have to wait until the weekend when I will have more time.
What I don't understand is how everything could have been working so well before I reformatted my HD and now it's all fubared...
Oh well, thanks for the help guys and perhaps it's time for me to start researching which 3D card to buy

Did you try disabling the audio and running it in OpenGL 3Dfx mode? It might still be a sound driver problem. You need to eliminate that possibility even if it seems unlikely.

Penguin,
yes I did. still got kicked. Is there any way that I can purge all traces of the old drivers from my system without having to go into the registry?
Perhaps there's something screwy in the autoexec.cfg I'm using. It is the old 3 Fingers High Performance cfg. that was writen for the V2 but I've always used it with this card and have never had a problem. I'll post it if you want to have a look:
set s_mixahead ".14"
set cl_predict "1"
set gun_x "-3"
set gun_y "0"
set gun_z "3"
set cl_particles "1"
set gl_dynamic "0"
set gl_polyblend "0"
set gl_playermip "2"
set gl_flashblend "0"
set gl_ext_swapinterval "1"
set gl_swapinterval "1"
set fov "110"
set cl_maxfps "60"
set rate "15000"
set gl_ztrick "1"
set cd_nocd "1"
set in_mouse "1"
set m_filter "1"
set in_joystick "0"
set gl_modulate "2.9"

Well you should definately wip the config to see if that's the problem. Backup your config.cfg and autoexec.cfg files and then delete them from your \QUAKE\ID1 folder. That will force Q2 to generate a new config.cfg file with default console settings.
