I had a 16 gig flash drive that was very slow, and was not recognized as having media. Using Manage, it could not be formatted or even partitioned. Tried a few utilities that did not work either. I finally fixed it by following these steps:

At first, it looked like it did nothing, but once I rebooted the pc, then I was able to partition and format the drive.

You can also run Spinrite level 2 on it to force it to swap out bad cells.
Just keep in mind that any flash drive that starts acting up should not be considered reliable for mission critical data/
